Assessment of pressure ulcer risk using the norton scale

Measurement of the risk of developing a pressure ulcer, through a validated scale.

Objective:

To quantify the risk of pressure ulcer presented by the patient.

Equipment:

Norton pressure ulcer risk assessment scale.

Material:

– Nursing records.

Procedure:

– Preserve the patient’s privacy. – Inform the patient. – Request the collaboration of the patient and family. – Assess the patient’s risk of pressure ulcer using the Norton scale. – Record in the nursing documentation: score obtained through the patient’s risk assessment, date and time.
